Tělo je v pořádku, ale mozek přesto vytváří bolest, která je ovšem skutečná. Jak funguje PRT? Východiskem léčby chronické bolesti dle PRT je stanovení, zda se jedná skutečně o bolest chronickou (v knize častěji nazývanou jako neuroplastickou), nebo o bolest strukturální (tělesnou, skutečnou apod.). PRT k tomuto účelu poskytuje sadu otázek ke stanovení původu a druhu bolesti. Vychází také z premisy, že chronická bolest je naučená, takže se ji lze i odnaučit. Významnou roli při vzniku chronické bolesti hraje strach, proto je řada technik zaměřena na vystoupení z bludného kruhu strach - bolest - strach. Stěžejní technikou Gordonovy metody je somatické stopování, k němuž poskytuje v knize metodické vedení. Dalšími častými technikami, které se v knize objevují, jsou například: korektivní zkušenost, vystavení se strachu, vyhýbavé chování, vysílání zpráv o bezpečí (uklidnění mozku), odkrývání hrozeb, hledání výjimek, sbírání důkazů, zachytávání obav a další.
