A New Vision for Women's Health Research - Engineering, and Medicine National Academies of Sciences, Board on Population Health and Public Health Prac
Women make up over half of the U.S. population, yet research on women's health conditions, including those that are female specific such as fibroids, more common among women such as anxiety, or affect women differently such as cardiovascular disease, is severely lacking. Medical advances for women have lagged, in part due to a lack of understanding of basic sex-based differences in physiology. To address this, the National Institutes of Health's (NIH) Office of Research in Women's Health tasked the National Academies with convening a committee of experts to assess the state of women's health research at NIH, identify critical knowledge gaps, assess the level of funding for women's health research, and more. The resulting report outlines specific recommendations for NIH women's health research priorities; training and education efforts to build, support, and maintain a robust women's health research workforce; improving internal structures, systems and processes; soliciting, reviewing, and supporting women's health research; and ensuring appropriate levels of funding.

In 2023, the National Academies convened an expert committee to assess the current use of racial and ethnic categories in biomedical research, review existing guidance for researchers, and provide new guidance for future use. The resulting 2025 report, Rethinking Race and Ethnicity in Biomedical Research, outlines nine actionable recommendations and associated resources for advancing the responsible use of race and ethnicity. The recommendations of Rethinking Race and Ethnicity in Biomedical Research address how to: decide whether to use race and ethnicity in different research contexts; characterize and disclose limitations of datasets that include racial and ethnic information; identify factors to investigate instead of or alongside race and ethnicity; include overlooked populations in analysis; and support sustained community engagement.

A lack of research into both the biological and social factors that influence these conditions greatly hinders diagnosis, treatment, and prevention efforts, thus contributing to poorer health outcomes for women and substantial costs to individuals and for society. The National Institutes of Health's Office of Research on Women's Health asked the National Academies of Sciences, Engineering, and Medicine to convene an expert committee to identify gaps in the science on chronic conditions that are specific to or predominantly impact women, or affect women differently, and propose a research agenda. The committee's report presents their conclusions and recommendations.

Artificial intelligence (AI) applications in the life sciences have the potential to enable advances in biological discovery and design at a faster pace and efficiency than is possible with classical experimental approaches alone. Although the creation of biological weapons is not a new concept or risk, the potential for AI-enabled biological tools to affect this risk has raised concerns during the past decade. This report, as requested by the Department of Defense, assesses how AI-enabled biological tools could uniquely impact biosecurity risk, and how advancements in such tools could also be used to mitigate these risks. The Age of AI in the Life Sciences reviews the capabilities of AI-enabled biological tools and can be used in conjunction with the 2018 National Academies report, Biodefense in the Age of Synthetic Biology, which sets out a framework for identifying the different risk factors associated with synthetic biology capabilities.

Chronic and episodic water shortages are becoming common in many regions of the United States, and population growth in water-scarce regions further compounds the challenges. Increasingly, alternative water sources such as graywater-untreated wastewater that does not include water from the toilet but generally includes water from bathroom sinks, showers, bathtubs, clothes washers, and laundry sinks- and stormwater-water from rainfall or snow that can be measured downstream in a pipe, culvert, or stream shortly after the precipitation event-are being viewed as resources to supplement scarce water supplies rather than as waste to be discharged as rapidly as possible. Graywater and stormwater can serve a range of non-potable uses, including irrigation, toilet flushing, washing, and cooling, although treatment may be needed. Stormwater may also be used to recharge groundwater, which may ultimately be tapped for potable use. In addition to providing additional sources of local water supply, harvesting stormwater has many potential benefits, including energy savings, pollution prevention, and reducing the impacts of urban development on urban streams. Similarly, the reuse of graywater can enhance water supply reliability and extend the capacity of existing wastewater systems in growing cities. Volcanic eruptions are common, with more than 50 volcanic eruptions in the United States alone in the past 31 years. These eruptions can have devastating economic and social consequences, even at great distances from the volcano. Fortunately many eruptions are preceded by unrest that can be detected using ground, airborne, and spaceborne instruments. Data from these instruments, combined with basic understanding of how volcanoes work, form the basis for forecasting eruptions—where, when, how big, how long, and the consequences. Accurate forecasts of the likelihood and magnitude of an eruption in a specified timeframe are rooted in a scientific understanding of the processes that govern the storage, ascent, and eruption of magma. Yet our understanding of volcanic systems is incomplete and biased by the limited number of volcanoes and eruption styles observed with advanced instrumentation. For more than 75 years, high-hazard structures in the U.S., including dams and nuclear power plants, have been engineered to withstand floods resulting from the most unlikely but possible precipitation, termed Probable Maximum Precipitation (PMP). Failure of any one of the more than 16,000 high-hazard dams and 50 nuclear power plants in the United States could result in the loss of life and impose significant economic losses and widespread environmental damage, especially under the pressures of climate change. While PMP estimates have provided useful guidance for designing critical infrastructure, weaknesses in the scientific foundations of PMP, combined with advances in understanding, observing, and modeling extreme storms, call for fundamental changes to the definition of PMP and the methods used to estimate it. Modernizing Probable Maximum Precipitation Estimation recommends a new definition of PMP and presents a vision for a methodology relevant for design, operation, and regulation of critical infrastructure. The new definition targets precipitation depths with an extremely low exceedance probability instead of assuming rainfall is bounded, and considers specified climate periods so that PMP estimates can change as the climate changes. In response to a congressional mandate, a committee of the National Academies of Sciences, Engineering, and Medicine is conducting a review of the Small Business Innovation Research (SBIR) and Small Business Technology Transfer (STTR) programs at the Department of Defense (DOD). These programs, created in 1982 and 1992, respectively, and operated within the DOD and a number of other agencies within the federal government, encourage the participation by small business concerns in the federal research and development and procurement processes through competitively based awards. As part of its review of the DOD's SBIR/STTR programs, and in response to a request by the DOD, the present study committee held a workshop on December 7-8, 2023, entitled "Data and Metrics for the DOD SBIR and STTR Programs." The workshop was convened to facilitate the DOD's development of recurring, quantifiable metrics for measuring the ability of the SBIR and STTR programs to deliver products and services that meet the DOD's mission needs, while also informing the National Academies committee's overall study of the programs at the DOD. Marine recreational fishing is a popular activity enjoyed by more than 9 million Americans annually and is a driver of the American ocean-or blue-economy. To ensure that fish populations are not overexploited, the NOAA Fisheries' Marine Recreational Information Program (MRIP) monitors recreational catch through a variety of in-person, telephone, mail-in, and other surveys. NOAA Fisheries' management of recreational catch also must take into account annual catch limits (ACLs) established to prevent overfishing for all managed species in federal waters. While MRIP has worked to improve recreational catch surveys over the past decade, the surveys were never designed to meet the demands of in-season management of ACLs. In some cases, estimates of harvest have triggered accountability measures such as early season closures and reductions in future recreational ACLs, which have been a source of contention with the recreational fishing community. This report presents approaches for optimizing MRIP data and complementary data for in-season management and considers alternatives for managing recreational fisheries with ACLs to better serve both social and economic management objectives.

A great deal of new research has been published during the last 15 years and there is a large amount of new information for many nutrients. Cost-effectiveness analysis (CEA) plays an important role in health policy debates, helping to shape resource allocation and pricing decisions. Yet many economists also recognize that the current framework can offer misleading and incomplete results. Current CEA methods imply that health improvements are equally valuable to those in good health and poor health, which fails to recognize the increased value of health improvements for those with severe illness or disability. Valuing Health introduces the generalized risk-adjusted cost-effectiveness (GRACE) model as a more accurate method for determining the value of medical treatments and technologies. The GRACE model generalizes the underlying CEA assumption of constant gains in health care, demonstrating through diminishing returns the greater economic value of improving the quality of life for individuals with disability or severe illness. Valuing Health also provides sensitivity analyses to show how value measurements change alongside key parameters, including the potential effects of various combinations of risk preferences on the aggregate value of treating a defined population with any set of available treatments.
