Od dubna 1920 se Kafka léčil z akutní tuberkulózy v jihotyrolském Meranu. V té době již byli spjati oba přátelé živou korespondencí, inspirovanou nejprve blízkými literárními zájmy. Po osobním setkání ve Vídni se Franz Kafka do Mileny zamiloval. Milující spisovatel se tu setkal s ženou inteligentní a chápající. Tomu nasvědčují už samy Kafkovy dopisy; v nich vedle čistě osobní linie, podávající analýzu vzájemného citového a milostného vztahu obou partnerů a zejména Kafkova nitra, se setkáváme s řadou filosofických, literárních, ojediněle i politických narážek, které dokazují, že okruh jejich společných zájmů byl velmi obsáhlý. Postupně se Kafkova nemoc beznadějně zhoršovala. V té době začal Kafka psát Zámek. Pravděpodobně setkání s Milenou přispělo ke vzniku jeho nejhlubšího, nejotřesnějšího, a jak mnozí přiznávají, nejrevolučnějšího díla. Nahrávka výběru z Kafkovy milostné korespondence vznikla v roce 1968. Dopisy Mileny Jesenské, Franze Kafky a Maxe Broda čtou Jaroslava Adamová, Radovan Lukavský a Martin Růžek s udivujícím vcítěním se do dramatických pocitů této význačné milenecké dvojice.
